Enrollment

LMMA provides care to children ages 6 weeks to 10 years of age.

We attempt to be flexible in our enrollment policies to suit the individual needs of our families. Therefore, we provide full and part-time registration. Of course, these options are accepted subject to availability of space and accommodations. We offer 3-day, 4-day, and 5-day enrollment and we offer part-time enrollment of less than 5 hours per day and full-time enrollment of a child for more than 5 hours per day.

*Tours of Facility are scheduled during business hours as scheduled with the Director.

To enroll a child at the center, the following forms must be downloaded and completed:

• Emergency Contact
• Enrollment Agreement
• Questionnaire
• Safe Routes Policy
• Acknowledgment of Video Recording
• IEP/IFSP Request
• Health Assessment
• Photo Release
• Supply List
